Well to start, I give this idea to anyone who need ...well an idea.
You could start as a Cell travelling trough a Body. You could chose a career like a Body defender, a nerves cell ect... and have quests like: Kill the SRAS bacteria or w/e. You can travel in blood, and reach many point like heart, brain. Energie could be used instead of money to buy DNA to change yourself.
It's a small idea open to everybody. It's more original than a rpg were you start in a village or a castle and you have to kick the big boss butt.

Just adding here:
You could also use some units flowing around as blood. Even though Doodads have no use here, add like...Rocks or whatnot.

It sounds interesting and unique, what tileset were you thinking of using? Or, any ideas for a tileset, since this is just an idea.

I'd say use a Ash World.

For The brain part Obviously Creep. Yeah for Veins maybe Ashworld but, Blood don't look like fire...
