Hmm. By realistic, do you simply mean something that one could practicaly do if given enough resources and money?
Being trapped in a large container full of water with a twisting and confusing path to get out, without knowing which way to go to find that path, or which direction is up, and having only a ten-second air supply with you. Oh, and no lights, of course.

Make them stand up without supports (just set them up in a way they cant sit down or fall over). Bolt their hands to a stool or something (or a wall, just so their arms are straight out)
Add a 1-lb. weight to their arm. After like 5 minutes put another on the other arm. Keep alternating arms for several hours or untill their skin starts to droop alot.
Eventually you may get enough weight to tear the skin.
Then take the weights off the fallen skin, and make them eats it!!1 YAY~~
(Also I was thinking maybe do this in another place... but I am not allowed to talk about that kind of things... hopefully you get it.)

Here's the situation, your stuck in a room, with a metal plate that is electrically charged, not enough to electricute you but to make your muscles hard to move. there is a platform with 5 tall glasses of water, which you must grab with both hands, on it, connected to a weight system and a timer. You must drink the glasses of water in 1 minute or less to make the weight system trigger the electrocution of the electric plate to stop. If the timer reaches 1 minute you will be electricuted. If you spill any water, it will conduct, you know the drill, fry like bacon.
