No, if you've already read it it will say "X new posts since you last read" whereas this says "You haven't read this thread yet".

Yeah, I think we've all noticed this one. It's from your cookies being cleaned since you've created the topic, or something has been reset somewhere.

I've seen that too. It usually happens if I haven't visited a topic in a couple of weeks. I guess something resets after that amount of time.
