Nothing says moderators HAVE to wait for everything to fill up. Warn levels really serve more as a guage and a tracking system for repeat offenders. People have been banned without hitting 100% because its obvious they have no intentions of change. Heck, there's even some crazy people who think its a good idea just to sign up so they can harass us with porn or something... they don't even get a warn.

There are lines that are hard to draw during debates. People have to keep reporting posts, I barely get any posts out of Serious Discussion (I don't think I've gotten one in over a week now).
Another problem is that a LOT of mods and staff keep going on outings and vacations, and it is becoming quite a nuisance. I hired MrrLL as a DLDB Keeper a few weeks ago simply because NOBODY was touching the DLDB backlog of over 50 files. The #1 problem with the DLDB is that there is almost no way to track a Keeper's activity, so I really have no idea who is active and who is not.

There is a repair function which all of the staff can use if the database goes down.

Well, those were my ideas. As I've said before (or didn't): you're too conservative for me. You don't want to try out new ideas.. maybe you think that they will hurt the site ? Maybe some will, but with experimenting, you can make the site actually better. You can't make an omelet without breaking the eggs.
A lot of limitations are in v4's code, which I simply dislike. I find Invision's code (particularly some of mods such as the DLDB/Store/etc) confusing, annoying, and poorly organized. I would like to try out all the crazy new rankings and such, but those are difficult to implement around here. v5 is a much better testing ground for things like that. On the subject of v5, I've been coding exclusively for v5 for awhile now. I like IP's coding style and find it very efficent and easy to understand.
