Hmm...
You know, I think the only official campaign I've ever completely won without cheating in any major computer game was the original Half-Life (and I only just did that yesterday). I've won a few small browser games, of course, but they hardly count.
EDIT: Come to think of it, I've also beaten Diablo II (killed Hell Baal that is, not gotten to level 99), and the first campaign in Heroes of Might and Magic III. And I won both the campaigns on the WarCraft II demo, if that counts. I wonder how I missed all those before. o.O

I unofficially hold the world speed record for beating Banjo-Kazooie (to the best of my knowledge). The offically recognized record is 2 hours, 55 minutes. A fellow at the same site claims (but has yet to post a video) of himself beating it in 2:48. My best time was 2:46, but I wasn't recording at the time, and I have so far been unable to beat 2:48 again (and haven't tried since summer).
I managed to do 100 consequtive super jumps in Super Mario RPG and obtained the Super Jacket, a rare feat indeed.
I've beaten many other games, but those are the rare and/or unique only to me accomplishments I've done in video games.

Mega Man X 100% run in 50:40 (current record - 39:41). Same game, run 100% with only the X-Buster in 1:05:00 (with an imaginable potential 50:00).

I have beaten every Resident Evil, ever. I am by far, the best over-all RE player.
I do not consider ANY RE that was not for a Nintendo system to be legitimate. RE is by Capcom, and Capcom loves Nintendo. RE: Out Break is a joke and a fraud.
Most notable achievements with RE:
RE2, beaten 31 times, in the course of 28 days. (Various stops inbetween, but all in all it was pretty constant. 3 hour sleep, eating while playing.)
RE3, beaten 3 times in 2 days, without stopping. (Bathroom/eating doesnt count.)
RE4, beaten in just over 13 hours, without getting hit once.
RE4, beaten the first time by my brother and I, in just over 13 hours. This is unheard of in non asians on their first try.
At an RE4 party, i smoked everyone in a mercenaries contest, blazing in with 5 combined matches racking up a score of a little over 3500000. My time for all combined matches was over 34 minutes. Thats true skill.
Other achievements:
I had a 19 hour Starcraft match. Map was Rise of the True Rome, and i was on the gundamn century mod. It was 1v1 and we both took all of the map before fighting. What a good match.
I had 22 consecutive matches where i BS'd, and won. The same person never came back to alert others as they usually do.
In a single night, from 11pm to 11am in the summer, i played 32 Phantom matches in a row, without getting Phantom once, and never losing.
I have beaten Zelda A Link to the Past on Super Nintendo more times then any human should ever have to do.
I am grand master of Mine Sweeper. Yes im that nerdy.

Uhm... Halo 2 on legendary, No co-op at that (dunno if thats impressive, I think it is tho) Beat Doa 2 (Very hard setting) and all costumes in close to 10 hours or just below... 100% on Morrowinds (Quest wise and ancient's wise)... Uhm... Those are the only things that are really impressive to me.
