Yeah, we need to agree on a policy for people at 100% warn.
IP suggested just leaving them there and punishing them even further once the limit was maxed.
Originally I liked the old reset to a 20x%, where x was the number of times they hit 100%.
I think I found a compromise - a period of probation whenever someone hits 100%.
If they don't get another offense during probation - they get the classic reset 20x% reset.
If they commit another offense on probation - they get a major punishment and a much longer probation period (and repeated offenses would be a ban).
The user hits 100%.
If most of the offenses were from a long time ago, then he's resest to 20X%.
Probation Period: Would be 2 weeks?
If the person breaks another rule, then add another 2 weeks?

Stuck at 100%... I'd say ban them. There's no reason to build up that kind of record.
I'd say maybe 20*2x%... is it really necessary to give people 6 chances?
